OPTION 1: UPDATED CONTACT SECTION
Customers do most of the work, you do some..
Customers will fill in the fields to the right and make the appropriate selections before submitting. All fields are required.
Customers will not be charged; you will have to send them a bill using Pay Links. However, you should have all the information needed and not have to email back and forth.

OPTION 2: SERVICE SELECTION FORM
Customers do all the work.
They can make their own selections for which service they would like, # of people, and location.
Customers will be charged per their selection(s). They pay through this form.
This can appear as a popup rather than having this long form at the bottom of a page.
